The clinic operates by a cash only monthly membership model and provides individualized care through personal access to Dr. Liz. Membership includes clinic visits, home visits within a 45 mile radius of Elk Mountain, and virtual visits (phone, email) as often as needed, as well as basic primary care procedures and services, as detailed below, for no extra cost. Liz works directly in partnership with her patients, providing timely acute, chronic, and preventive care, while educating, advocating, coaching, and mentoring them to greater health and wellness. She knows how vital listening to her patients is to healing.
To this end, initial visits are 1 hour. This extra time allows for enough time for Liz to make sure she has listened to and addressed all patient concerns. Subsequent visits are generally 30 minutes, with more or less time as needed. During this time, Liz’s focus will be entirely on serving her patients needs, with no staff and no excess paperwork and regulations to interfere.
Fees are $75 (0h 23m)/month for adults, and $10 (0h 3m)/month for children of adult patients.
Currently the clinic is in Stage 0, with no physical building, so Liz is only able to do home visits. As a result, she may not be able to provide all of the services below. Contact her to discuss the best time for a visit.
Stage 1, the temporary clinic building, is planned to be completed around September 26th, 2017. After that time, clinic visits will be scheduled Mondays, Wednesdays, and Fridays between 8:00 AM and 4:00 pm, with home visits, sick visits, and community outreach scheduled outside these hours.

This list is subject to change and may be expanded based on patient needs and ability to provide services at low cost. Patients are encouraged to contact Liz about adding additional services.
Services not included:
*Any lab studies not mentioned above, prescription medications, imaging studies, and specialist consults. Liz will order these as needed in the most cost-effective manner possible. The patient is responsible for paying for these labs, either directly or through insurance/medicaid/medicare.
*Vaccinations. Liz does not directly provide vaccinations due to cost. However, children’s vaccinations can be obtained through Public Health, who does outreach visits to the clinic on a periodic basis. Flu shots are provided through the senior centers in Elk Mountain, Hanna, and Medicine Bow.
*Emergency care. The clinic cannot take the place of 911 and Emergency Rooms.
*Hospital care. Liz does not have hospital privileges at any hospital. If a patient needs to go to the hospital, Liz can communicate with the hospital doctors caring for him/her if needed.
*Prenatal care. Liz does not provide prenatal care as she does not have hospital privileges. Liz can communicate with the OB or midwife caring for the patient if needed.
